Psilocybin Therapy Shows Five Year Benefits for Depression

Could psilocybin-assisted therapy provide lasting relief from major depression? A new study published in the Journal of Psychedelic Studies suggests the answer might be yes, with results extending five years beyond treatment. 
The implications are hopeful and potentially transformative.

A First-of-Its-Kind Long-Term Follow-Up

Researchers followed participants from an earlier clinical trial of psilocybin-assisted therapy (PAT) for Major Depressive Disorder (MDD). Twenty-four adults who previously completed the trial were contacted, with 21 enrolling in the long-term follow-up. Over five years, the study tracked depression severity, anxiety, functional impairment, and the participants’ lived experiences.

While previous studies had shown short-term benefits of psilocybin for depression, this is the first study to explore outcomes five years after treatment.

The Results: Rapid and Sustained Improvements

Depression Relief: Participants experienced a significant reduction in depressive symptoms. At five years post-treatment, 67% were in remission, with scores largely unchanged from those measured shortly after the initial therapy sessions.

Anxiety and Functioning: Anxiety and global functioning also improved. Participants reported better work, social, and family functioning, and overall daily life became more manageable.

Safety: No severe or lasting adverse events were linked to the therapy. Some participants experienced brief periods of heightened emotional sensitivity, but these were transient and manageable with support.

The Lasting Impact on Mindset and Relationships

Beyond measurable symptom relief, participants reported meaningful changes in how they relate to themselves and others. Many described increased empathy, self-acceptance, confidence, and emotional awareness. Relationships improved, and some developed healthier boundaries and habits that supported long-term wellbeing.

Interestingly, even participants who later used antidepressants or psychotherapy reported that their experience with psilocybin shaped how they approached their mental health, often reducing the severity of future depressive episodes.

Why This Matters

Depression is often chronic, recurrent, and resistant to standard treatments like SSRIs or therapy alone. Traditional antidepressants can take weeks to work, may cause side effects, and require ongoing daily use. Psilocybin, in contrast, offers a rapid and profound experience that may alter patterns of thinking and emotion in ways that support lasting recovery.

Researchers believe the combination of guided therapy and the psychedelic experience itself is central to its effectiveness. Participants described shifts in perspective, emotional insight, and a deeper sense of meaning, experiences that can fundamentally change how someone relates to their depression.

This was a relatively small study, so larger trials are needed to confirm these results. Future research will also explore the mechanisms that make psilocybin-assisted therapy so enduring and how it can best be integrated with other forms of mental health care
